Mission Australia – NDIS Local Area Coordination – South East Metro Perth

Mission Australia is committed to delivering the Local Area Coordination (LAC) as part of the National Disability Insurance Scheme (NDIS) across Western Australia.

The National Disability Insurance Scheme (NDIS) allows Australians with a permanent and significant disability the freedom to choose the life they want to live. The scheme helps people with a disability to access the supports that are right for them and assists in building their skills to actively participate in their community.

Local Area Coordinators work with people with disability and their families. They can help you to understand and access the NDIS and link you to information and support in your community.

Child and Adolescent Mental Health Service - Emergency Telehealth Service

Urgent mental health telephone and online videocall support children and young people in the metropolitan area that are experiencing a mental health crisis. Support and advice is also available for families, carers and health professionals. The service is available from 8am until 2.30am daily.

MIFWA – Early intervention Recovery Program

The MIFWA Early Intervention Recovery Program (EIRP) is for young people aged 16 to 30 years who have been diagnosed with a mental illness including first episode of psychosis.

Operating across the Perth Metropolitan area, our EIRP team supports people to re-establish or maintain social, professional and educational networks.

The program is initially for 6 months but can continue for up to one year.

MIFWA understand each individual’s mental health journey is different, so they tailor their supports around your needs, aspirations, goals and recovery. The EIRP team provides one-on-one support in your own home and local community, working with you to set goals and reconnect with your priorities.

Ruah EPYCentre

Ruah has extensive experience supporting young people, particularly those with severe mental illness, and its EPYCentre is a specialist functional recovery service for young people aged 12 to 25 years.

Professional staff support clients and their families from across Perth’s southern suburbs who are referred by mental health professionals. Staff walk alongside clients assisting them to develop a personal recovery plan incorporating life goals, education, employment, recreation and fitness or manage stress and build social connections. The service is also available to young people experiencing LGBTI+, transcultural and other disadvantage issues.

Based in Cockburn, it is a welcoming environment where young people who have experienced psychosis can plan their own recovery.

Families - The EPYCentre provides recovery-oriented support to help young people on their recovery journey. This can be a difficult time and family plays an important part in the process.

Our Family Peer Workers, who have lived experience, are available for one-to-one family support. Groups are run with other families and loved ones who can share their experience and advice.

Any State Area Health Service Provider in Perth’s southern suburbs can refer a young person to the EYPCentre. You are welcome to contact us for more information or to arrange a visit.

Child and Adolescent Mental Health Service – East Metropolitan Youth Unit (EMyU)

The EMyU is a 12-bed inpatient mental health service for young people aged 16 to 24. It provides care for youth presenting with complex and acute mental health issues. These include psychosis, depression, severe anxiety, post-traumatic stress, risk of self-harm or suicide and emerging personality disorder. The service has a focus on early episode interventions.

EMyU provides 24-hour, seven-day-a-week inpatient assessment, treatment and community care planning by a multidisciplinary team for youth experiencing complex and severe mental health problems.

Bentley Health Service – Early Intervention in Psychosis (EIPS)

The EIPs service provides community-based intensive individualised and comprehensive care for young people with psychosis and their families. This recovery focussed service  is available to patients aged between 16 and 35 years of age who have been diagnosed with a first-episode of psychosis.

East metro health service – Assessment and Treatment Team

The Assessment and Treatment team (ATT) is the first point of contact for all referrals to adult community mental health services within the City East catchment area.

The multi-disciplinary team includes psychiatrists, nurses and allied health staff who help to provide triage, assessment and short-term care and treatment options for consumers up to a maximum of 10 weeks.

East metro health service – Clinical treatment team

The Clinical Treatment Team (CTT) receives all referrals via the ATT and offers multi-disciplinary mental health assessment and care coordination for consumers who require specialist care and treatment options for more than 10 weeks.

Older Adult Mental Health Service

The Older Adult Mental Health Service provides a comprehensive recovery-focused mental health service to older adult consumers within the inner city and eastern metropolitan region of Perth.

The service provides an assessment and/or case management service to consumers aged 65 or over (45 for Aboriginal and Torres Strait Islander consumers) who have mental health issues, or who are experiencing severe behavioural and psychological problems associated with dementia. The team consists of psychiatrists, community nurses, social workers, occupational therapists and psychologists.

Richmond Wellbeing - National Psychosocial Support Measures Program

The National Psychosocial Support Measures Program (NPSM) provides time-limited supports to eligible individuals that need to develop stability in personal, social, professional and educational areas.

An NDIS eligibility decision is not required for new applicants to be eligible to receive psychosocial support and individuals may be supported to make an NDIS eligibility application as part of their program.

Richmond Wellbeing - MH Connext

The MH Connext nursing case management program is available for people 18 years and older with severe clinical mental health symptoms currently being managed by a GP. Mental Health Nurses support a patient’s needs as part of their Mental Health Treatment Plan(MHTP) provided by their GP. This free program is provided during business hours for between three to six months. It does not provide psychological interventions and can be in addition to other psychological treatment.

To meet the criteria for this service, patients must also have one of the following:

  • Co-existing conditions such as chronic conditions

  • Multiple social stressors such as housing, finances/Centrelink, social support

  • Alcohol and other drug support (including harm minimisation)

Richmond Wellbeing’s Individualised Community Living Strategy (ICLS) team works with you in providing support that assists you to achieve your long-term goals. They assist you to live independently in your own home, access the community and develop meaningful relationships. They will work with you, your family, your mental health services, housing services and other significant people or groups in your life, to develop a recovery plan tailored to your goals.

Life Without Barriers Individualised Community Living Strategy (ICLS) Program

Supports people aged 18-65 years with complex mental health challenges to achieve their recovery goals and live well in the community.

Life Without Barriers is a provider of the Individualised Community Living Strategy (ICLS) program. ICLS takes a holistic approach to wellness by delivering safe services, encouraging recovery and patient creativity, challenging stigma and inspiring hope.

Life Without Barriers can only accept referrals to the ICLS program from the Mental Health Commission of WA who fund the program.

Ruah - Choices

Choices is a personalised peer support and case management program which supports vulnerable people as they are leaving Royal Perth Hospital Emergency Department or the Courts. This short-term program connects people to primary and secondary care and community support services aligned with their individual needs to avoid hospital readmission.

Aboriginal Mental Health Practitioners support Aboriginal and Torres Strait Islander people and their families who are affected by mental health, alcohol and other drugs issues. A cultural model is used which provides a holistic and strengths-based approach to recovery.

Wangen Kartup Specialist Aboriginal Mental Health Service

The Wungen Kartup Specialist Aboriginal Mental Health Service supports both Aboriginal consumers and carers in accessing mainstream mental health services, and in better meeting the needs of Aboriginal people.

This Adult Mental Health service provides advocacy for Aboriginal consumers, and operates as a peak body for Aboriginal mental health professionals.

Life Without Barriers Ngatti House

Supports people aged between 17 and 22 years, who have a diagnosed mental illness or are at significant risk of developing a long-term mental illness, are homeless or at risk of long-term homelessness, and require medium to high-level support.

Based in Fremantle, Ngatti House is a purpose-built rehabilitation facility that offers safe and secure accommodation for 17-22 year olds looking to work on their mental health recovery. The house is staffed 24 hours a day with caring and skilled workers who work alongside residents to imagine a meaningful future, identify goals they would like to work towards and how to use their strengths and skills to live well.

Neami National Mental Health Step Up/ Step Down Service - Joondalup

A free short-term residential support service for people with a mental illness to prevent hospital admission or support early discharge. It is available to anyone living in WA with a mental illness diagnosis.
